The USMLE Step 1 Exam is an exam consisting of questions provided by faculty members of medical schools located in the United States of America, where basic medical sciences are tested.
Application, Timing and Costs
To apply for the USMLE exams, you must submit an application through the appropriate registrar. Applications cannot be made during the first 14 days of the year and local holidays. These institutions and the Istanbul Doctors Academy team will support you in the application process so that you do not have any problems.
Application fees start from $975 for medical school students and graduates who will take the exam outside of Canada and the USA. Although it is separate for each stage, there are separate costs for certification, CV verification and mandatory forms. It can be changed free of charge 46 days before the exam. In case of not being able to enter due to weather conditions or justifiable reason on the exam day, it must be reported to the necessary official authorities.
You can take the exam 4 times per step. If you fail all 4 exams, you cannot apply for the steps of the USMLE exams.

What is the USMLE policy on test entry limits? The total number of attempts allowed per step is four (4).
If you have attempted and failed a Step four or more times, including missing attempts, you will not be able to apply for any Step in the USMLE exam series.
The USMLE Composites Committee, the governing body of USMLE, considers all aspects of the exam as it works to ensure the program fulfills its mission of supporting medical licensing authorities in the United States. To preserve the integrity of the exam, the committee voted to change the number of attempts allowed and to be closer to the USMLE initiative limits imposed by the majority of state medical boards.
As part of the review, the committee was also presented with information showing that it is rare for people who take more than four repeat attempts at the USMLE Steps to successfully complete the exam sequence, gain access to graduate education, and ultimately be licensed to practice medicine.
Policy change applies to all applications submitted on or after July 1, 2021
If you have a documented disability under the Americans with Disabilities Act (ADA) and need adjustments, you should review the procedures and submit your application for the exam and your test placement request at the same time as the required documents.
Biometric Information
Biometric registration includes some or all of the following information:
a digitized representation of your fingerprint (template), passport
an image of your fingerprint along with your name, address, phone number (if applicable)
date or age of birth, identity document number
a scanned copy of your identity document (your driver’s license, passport, etc.) containing your digitized photo
The biometrics record is matched with credentials and information about your test appointment.
Unless required by law, personal information collected using biometrics (including your fingerprint image and template) is stored on behalf of the USMLE program sponsors in a secure, centralized database in the United States on the day of the test and for a period of 5 years from the date of the test.
Access to your biometric information will be limited as described in the Privacy Notice for USMLE Candidates. Employees or representatives of USMLE sponsors who have access to data are subject to confidentiality agreements. Data may be made available to third parties if required by law or necessary to complete a test fraud investigation directly related to the relevant candidate(s).
USMLE Step 1 General Questions
January 26, 2022 is the effective date of the change. All Step 1 exams taken on or before January 25, 2022 will receive a numerical score and pass/fail result. All Step 1 exams taken on or after January 26, 2022 will receive a pass/fail result only.
All USMLE transcripts, including those sent through the Electronic Residency Application Service® (ERAS®), will contain the same information. A Step 1 exam taken on or after January 26, 2022 will only appear on transcripts as a pass/fail result without a numerical score. Step 1 exam taken on or before January 25, 2022 will appear on transcripts as numerical score and pass/fail.
The primary purpose of the USMLE is for licensing. USMLE co-sponsors are aware that there are other uses for the USMLE, such as residency selection, but an ideal system for evaluating candidates should be holistic. This system should include various assessments of the candidate’s competencies and unique characteristics. In such a system, the USMLE will function as a single objective measure and will be appropriately weighted.
New content was not evaluated in the Step 1 exams administered after the October 2020 exam was updated. After the October 2020 update, test takers’ tests saw an increase in items assessing Communication and Interpersonal Skills; this content has been included in the Step 1 exam for many years.
Step 1 pharmacology items continue to focus on exam takers’ understanding of drug mechanisms, rather than pharmacotherapy. Step 1 test takers are not required to identify specific drugs; However, those who take the Step 2 CK exams are asked to do so.
USMLE Step 2 CK General Questions
Questions that evaluate basic science concepts are addressed in Step 1. However, basic science knowledge will often be needed to answer the items in Step 2 CK, even if the question is focused on diagnosis or management.
Test takers who tested before the exam was updated in November 2020 saw slightly more Diagnostic questions than the Administration items in Step 2 CK, while test takers after the update saw an equal amount of both types of test items. The proportion of questions allocated within Organ Systems remained unchanged.
USMLE Step 3 General Questions
You must act as a primary care physician and take responsibility for the patient in all management positions throughout each case.
Do not assume that when a patient is admitted to a facility or transferred for a surgical procedure, other members of the healthcare team (for example, nurses, medical advisers) will write or initiate instructions for you. Responsibility for the primary care of the patient may include (but is not limited to):
Ordering lab tests, imaging studies, and diagnostic procedures
Changing management locations when necessary
Starting and changing treatment regimens
Dealing with life-threatening emergencies
Monitoring the patient’s condition during the simulated time
Prepared patient care that can endanger the patient if neglected
(for example, in the pre-operative setting this may mean asking for IV fluids, blood type and cross-match, and antibiotics)
Addressing health care issues.
Note: The nature of each case determines whether health care issues are relevant within the simulated time frame.
When the case is over, your responsibilities for each patient are fulfilled.
In case of a computer problem during the instant inspection, notify the supervisor. A case can be restarted by test center personnel. Only one reboot is allowed per case. If a case is restarted more than once, the restart restriction will prevent the interrupted case from completing and the next case will appear.
Exam Scoring
Step 1, Step 2 and Step 3 exam results are announced within 3-4 weeks.
In case of computer problem, notify supervisor. A case can be restarted by test center personnel. Only one reboot is allowed per case. If a case is restarted more than once, the restart restriction will prevent the interrupted case from completing and the next case will appear.
